Annual Health and Medical Record
A completed Annual Health and Medical Form is required for all scouts. Only Parts A and C are required for Cub Scouts. Completed forms should be sealed in an envelope and given to a Pack 40 leader. The record will be held by the Cub Master and will only be used in the event of a medical emergency.
